MEDICAL STAFF APPLICATION
Thank you for your interest in applying for the Foothill Presbyterian Hospital Medical Staff. 

The Medical Staff Application process is divided into five sections.  Please go to each step and follow the directions carefully.

1. Application - Download the Medical Staff Application form
2. Application Fee - The non-fundable fee for applying to the FPH Medical Staff is $550.00.  We accept checks payable to Foothill Presbyterian Hospital.  Annual dues are $175.00.
3. Privilege Forms - Please locate the appropriate Privilege Form on the Privilege Forms Page.
4. Training Modules (Please contact the Medical Staff office to request Procedural Sedation Privileges)
5. Mailing Instructions - Please mail signed Medical Staff Application, signed Medical Privileges Form and application fee to:

PROCESSING TIME
Our standing processing time is 30-90 days following receipt of a completed application.
